#PremiseCares: Premise’s care navigation program provides comfort to COVID-19 positive patient

When one Premise Health member presented with a positive COVID-19 diagnosis, licensed professional counselor, Kate Lifferth, stepped in to provide much needed comfort. During a virtual visit, the member repeatedly noted her concern and anxiety regarding the diagnosis and how it would impact her family. Following the appointment, Kate reached out to the onsite health center manager to ask about enrolling the member in Premise’s care navigation program. Once enrolled in the program, both Kate and the health center manager spoke with the member on a daily basis to ensure she was coping appropriately with the home isolation and her symptoms were not worsening.

Thanks to the care and support of Kate and the entire health center team, the member was able to effectively manage her diagnosis and stay physically and mentally healthy in quarantine. Well done, Kate!
