#PremiseCares: Premise providers get a member’s health back on track

Recently, one member presented to the Premise Health wellness center after not having access to a primary care provider for over a year due to not having insurance. During his first visit, our providers immediately wanted to check to ensure his blood pressure was controlled and work to get him any needed medication refills. In addition, the providers also identified a need for mental health support and additional treatment for diagnosed issues that had gone untreated. Premise providers Michele Sherrill, FNP-BC and Jennifer James, MA developed a plan of care for his diagnoses and made the appropriate referrals for his needs. While getting established with the specialists, the member continues to have follow-up visits every month with his Premise providers.
