Throughout the coronavirus pandemic, Premise pharmacists have used proactive outreach to inform members of our services and provide them convenient and cost-effective care. One pharmacist called a team member who had been using her community pharmacy instead and introduced her to our Sync Your Care program. Sync Your Care allows our pharmacists to better track a members’ medications and sync their date and fill amounts to make prescriptions more manageable. While SYNCing this new member, the pharmacist realized she had been prescribed two low-dose medications for the same symptoms.
After consulting with the patient’s provider, it was confirmed that this was an error and the patient was not meant to be on both medications simultaneously. Our pharmacist was able to remove this prescription from her routine intake and save the patient an additional five dollars per month. This correction also saved the patient’s company from covering an expensive brand name medication each month.
