Throughout COVID-19, Premise Health pharmacists have been conducting daily calls to members to check-in on their wellbeing, help transfer prescriptions, and stay connected. One Premise Health pharmacist called a member with a known history of diabetes to check in with how she was managing her condition during the pandemic. The member noted that she had been experiencing high sugar levels but was unable to see a specialist in order to receive new medication. After walking through a series of questions over a virtual pharmacy visit, the pharmacist suggested that the added stress of recent changes at work and the current pandemic environment was likely contributing to her high glucose readings.
After a follow-up call several days later, the member expressed that she had started to be more active again, was being mindful of what she was eating, had lowered her blood sugar, and overall was starting to have a much better outlook on her condition. Without the continuity of care that this Premise pharmacist was able to provide, the member easily could have lost control of her condition.
