What it means to be a member.
Your experience matters.
We’re not your typical doctor’s office. We deliver a welcoming experience from the moment you walk in the door and provide care that is inclusive to all races, religions, countries of origin, sexual orientations, gender identities and abilities. You belong here.

Your membership is covered.
Members are what we call those who are eligible to use our services. You’re considered a member if your employer has partnered with Premise Health to offer you and your family care at this location. No membership fees. No application. Schedule a visit to get started.
The cost is clear.
Whether free, covered by your insurance, or available for a set fee, we offer many of our services at a lower price than what you’d find elsewhere in the community. We’re able to tell you what a visit is going to cost upfront, so there are no surprises.
Our care is convenient.
We offer same- and next-day appointments and short wait times so you can be seen quickly. You can also meet with a provider virtually from the comfort of home. Plus, the My Premise Health app makes it easy to schedule appointments and communicate between visits.
You’re in good hands.
Our care is personal and dedicated. We’re not open to the public like other community clinics, which means our providers spend more time getting to know you to understand your health needs. We aim to build a trusting relationship with you so we can deliver better care.
