Primary Care
Pediatric Care (ages 2+)
Whether your child has a sore throat or needs a sports physical, you can depend on us to support their health and wellbeing. Your children have access to the same services available to you, from providers you can trust.
Behavioral Health
Your mental health matters. With in-person and virtual counseling, you can see licensed professionals who use counseling techniques and evidence-based talk therapy to help with anxiety, stress, grief, depression, and more.
Lab Services
We offer onsite testing and offsite collections and can send your results electronically through My Premise Health. You can also discuss your results with your provider during an in-person or virtual follow-up appointment.
Women’s Health
A woman’s healthcare needs are unique. That’s why we give all women, and those assigned female at birth, the personalized care they need to stay well. Our providers also offer screenings for breast cancer and cervical cancer.
Virtual Visits
If you’re at home or don’t feel well enough to stop by, you can get the care you need. The Premise Health Wellness Center offers virtual visits through your patient portal, My Premise Health, during business hours.
Physical Therapy
Don’t just live with your aches and pains. Through a personalized treatment plan, our physical therapists can help identify the root cause of your pain, work with you to prevent future injuries, and get you back to moving with ease.
Provider-dispensed Medications
The Premise Health Wellness Center conveniently carries many medications for your needs. That way, you can get your prescriptions from your provider during your visit and skip the extra trip to the pharmacy.
