What is Premise Health?

Premise Health is the organization that will operate and staff the Carpenters Center for Health. They are one of the nation’s leading managers of onsite health clinics. Premise Health has cared for thousands of people, serving some of the nation’s top companies at more than 550 locations, and has more than 4,600 team members.

Premise Health will help keep you at your best – at work, at home and everywhere you go; they do this with experience, and shifting the focus from treatment to prevention, empowering you to make health and wellness a priority.

What sets Premise Health apart?

The cornerstone of the Premise Health experience is simple: it’s all about you. Our physicians and staff are focused on your wellness. Compared to traditional community medical practices, our providers spend more time building an understanding of your health. It’s a truly different experience. It’s less rushed. And it starts with having the right people to take care of you. People who will listen. People who will build a real, trusting relationship with you – leading to better care.

What is a patient centered medical home?

A patient centered medical home (PCMH) is not a building, a “home”, or a hospital, but rather an approach to providing you and your family comprehensive healthcare. It’s a powerful new effort focused on treating you, the “patient”, and giving you the best care at all times. The PCMH model is all about strengthening the provider-patient relationship; providing whole-person, coordinated care, enhancing the healthcare experience and outcomes through better communication, expanding hours and access, and encouraging active patient participation in healthcare delivery. At the Carpenters Center for Health, you will be surrounded by a dedicated team of health professionals working together to meet all of your individual healthcare needs. The Health Center’s goal is to make it easy and comfortable to get the care you need in a way that works best for you, while delivering the highest quality of healthcare.

Will I have to use the Health Center?

Receiving care at the Health Center is completely voluntary. The choice is yours. You can continue to receive care from your regular doctor if you prefer but the Health Center will be a convenient, low to no cost option. If you choose to receive primary care services at the Health Center but need a referral to a specialist, the staff will help connect you to an appropriate in-network specialist for your health condition.


What types of healthcare services will the Health Center offer?

The Health Center is a patient centered medical home that will provide comprehensive services focused on strengthening the provider-patient relationship, providing whole-person, coordinated care. Services will include the following:

  • Primary Care Services
    • Annual physicals
    • Lipid profiles, blood glucose and blood pressure screenings
    • Flu shots, immunizations and vaccinations
    • School and sports physicals
    • Skin, breast and prostate cancer screenings
    • Allergy management
    • Wellness exams
  • Acute/Urgent Care
    • Common cold
    • Sinus infection and stuffy nose
    • Seasonal allergies
    • Minor surgical procedures
  • Disease/Condition Management
    • Diabetes, high cholesterol, hypertension
    • Asthma, COPD
    • Back pain, migraine, osteoarthritis
    • Depression, stress, obesity/overweight
    • Limited generic medications will be provided on site as part of your visit with the provider in support of condition management
  • Population Health and Patient Education
    • Nutrition
    • Smoking cessation
    • Drug and alcohol awareness
  • Referral management

How will my current medical provider know I am receiving care at the Health Center?

After you have been treated at the Health Center, you may request any records be sent to another medical provider.  If your community provider uses Epic (or My Chart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.

Who will be administering my care?

Your Premise Health center staff includes a full-time physician and Walgreens pharmacist, as well as a nurse practitioner, wellness nurse, physical therapist and medical support personnel. Everyone on your center’s team has been fully screened and selected by Premise Health for their leading-edge knowledge and skills. The Premise Health onsite health center is not designed to replace your current doctor. It works alongside your current medical team, adding a new layer of convenience so you never miss a beat. Plus, if you ever need a referral to a specialist, we’re right here for you.


How much will it cost to use the Health Center?

All services provided at the Health Center are dependent upon your organization’s benefits plan.


Who is eligible to use Health Center?

Access to the Health Center will be available to members. For some organizations, this includes spouses and dependent children, ages 2 and up who are eligible and enrolled in comprehensive medical benefits.

Can I use the Health Center if I’m not currently eligible for benefits?

No.  To use the Health Center, you and your dependents must be eligible for benefits and enrolled in comprehensive medical benefits.

Can I continue to use the Health Center if I’m not eligible for benefits but I’m willing to pay for services?

If you are not eligible for benefits, you may not use the Health Center.


How will I make an appointment at the Health Center?

When the Health Center opens, you can call or visit the center during regular hours of operation to make an appointment.  There will also be an online patient portal where you can make your appointments via your computer, phone or tablet. Will the Health Center accept walk-ins?

Walk-in patients are welcome, but it’s always preferable to schedule an appointment.  In most cases, same-day appointments are available.

What is My Premise Health, the electronic health records (EHR) platform?

The EHR and patient portal platform is Epic, the nation’s leading ambulatory electronic medical record provider for nearly 20 years.  Epic offers a fully integrated suite of leading technology products, has the distinction of being the #1 Overall Software Suite in KLAS, and 50% of the U.S. health systems are actively using the Epic network.  It is referred to as My Premise Health.

What is My Premise Health?

My Premise Health will be your new patient portal which is part of the Epic’s electronic health records platform.  My Premise Health is a secure online and mobile tool that will put your health information in your hands.  My Premise Health will connect you with your healthcare experience by providing convenient access to book and manage appointments, message your providers, conduct virtual visits, view visit history, test results, and more. What will I be able to do with My Premise Health?

• Conduct virtual visits

• View your immunization history and health reminders

Will my health information be secure? Who can access to my personal health information?

Your personal health information is confidential and your data will be kept secure.  The Health Center will operate in accordance with HIPAA and work diligently to protect all health records.  Premise Health cannot share any personal health information without your permission. Rest assured, your personal health information will not be shared with your employer.  Additionally, Premise Health will operate through a private network and software systems that cannot be accessed by other members or company staff.  Notice of privacy practices will be available for review within the Health Center.
