Will I have to use the Wellness Center?
Receiving care at Premise Health is completely voluntary. The choice is yours. You can continue to receive care from your regular provider if you prefer, but Premise Health is a convenience provided to you by your employer. If you choose to receive primary care services at Premise Health but need a referral to a specialist, the staff will help connect you to an appropriate in-network specialist for your specific need.
What types of healthcare services are offered at Premise Health?
While products at each Premise Health location may vary, we will always remain focused on strengthening the provider-member relationship and providing whole-person, coordinated care. Products may include the following:

How will my current medical provider know I am receiving care at Premise Health?
After you have been treated at the Premise Health, you may request any records be sent to another medical provider. If your community provider uses Epic for their electronic health records (EHR), your information will be available via the My Premise Health platform and vice versa.
What type of providers will I be able to see?
Your Premise Health location may be staffed by a variety of healthcare professionals depending on your organization’s specific needs. This may include a physician, physician assistant, nurse practitioner, pharmacist, wellness nurse, physical therapist, and other medical support personnel. Premise Health maintains the same education and licensure requirements as any other medical facility, and you can be confident that your providers have been fully screened and selected for their leading-edge knowledge and skills.
How much does it cost to see a provider at a Premise Health facility?
The cost structure for all services provided by Premise Health are dependent upon your organization’s benefits plan and your individual plan selections.
Who is eligible to use services from Premise Health?
Access to Premise Health is available exclusively to members. For some organizations, this includes spouses and dependent children, ages 2 and up. Eligibility will vary by organization and may require being enrolled in your organization’s benefits plan.
How will I make an appointment at Premise Health?
We pride ourselves on our quality and efficiency, and that includes the way we schedule appointments. We offer a convenient, easy-to-use app – My Premise Health – or you may call your Premise Heath location and speak to one of our friendly team members for assistance. We can often offer same-day appointments with minimal wait times.
Does Premise Health accept walk-ins?
Policies can vary depending on location, so it’s best to check with your site. Keep in mind that in most cases, same-day appointments are available.
What is My Premise Health?
My Premise Health is your member portal. It is a secure online and mobile tool that puts your health information in your hands. My Premise Health connects you with your healthcare experience by providing convenient access to book and manage appointments, message your providers, conduct virtual visits, view visit history, see test results, pay your bill, and more.

What is an electronic health records (EHR) platform?
Premise Health partners with Epic, the nation’s leading ambulatory electronic medical record provider, to maintain a digital version of what we used to know as paper charts. EHRs are real-time, patient-centered records that make information available instantly and securely to authorized users. The system also allows any provider who uses Epic to access your file and helps them make decisions about your care.
Will my health information be secure? Who has access to my personal health information?
Your personal health information is confidential, and your data will be kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. Premise Health cannot share any personal health information without your permission unless it pertains to a work-related, occupational health injury. Rest assured, your personal health information will not be shared with your employer. Additionally, Premise Health will operate through a private network and software systems that cannot be accessed by other members or company staff. Notice of privacy practices is available for review within your local Premise Health.
